The SELLER shall deliver to Buyer a written five-year insurance claims history regarding
Premises (or a claims history for the length of time Seller has owned the Premises if less
than 5 years) from Sellers insurance company or an insurance support organization or
consumer reporting agency.
If you follow the instruction below you should be able to pull up your report in a few
minutes for free (Once every 12 months). Should you have trouble with this please call 1-
866-312-8076 or contact your insurance provider for a report. Your provider can fax a
report to us at 480-892-4759.

REPORT GO TO: WWW.CHOICETRUST.COM- Click on CLUE report (a Lexis, Nexis service)
- Click on Clue Reports in compliance with FACT Act
- Select the CLUE 'Personal Property Report', click on order now
- Sign In Create a log in ID & Password
- Enter Property Information
- Enter Your Birth Date and Social Security #.
- Verify Information
Print Report & Fax to us at 480-892-4759. (Make sure your social security is not showing on printout)
The C.L.U.E. ®Personal Property report provides a five-year history of losses
associated with an individual and his/her personal property. The following data will be
identified for each loss: date of loss, loss type, and amount paid along with general
information such as policy number, claim number and insurance company name.
